PHOTOGRAPHY/ADVERTISING WAIVER:

I hereby permit Amy Beccari Dance Studios to use photographs (and other media forms) which may contain my child’s image, for advertising and promotional purposes.

DISCLAIMER:

My child dances at regular classes, competitions and concerts at their own risk. I declare to not hold Amy Beccari Dance Studios liable for any injury sustained to my child, loss of life and loss or damage to property whilst enrolled at Amy Beccari Dance Studios.

CHOREOGRAPHY:

No choreography whilst enrolled is to be performed or duplicated in any way or formwithout written permission from Amy Beccari Dance Studios.

CANCELLING ENROLMENT:

If my child is to leave Amy Beccari Dance Studios I will inform the proprietor andagree to pay any outstanding lesson payments.

SUPERVISION OF CHILDREN:

Amy Beccari Dance Studios teachers supervise students in their enrolled class time only. All children whilst at ADBS studio not in their dance class MUST be supervised by an adult at all times.

ABDS Code of Conduct

I have read ABDS Code of Conduct listed in the Info Guide and understand the terms.

Covid Policy

Credit for classes will be available if ABDS can not provide any options to run any type of classes during covid lockdowns or restrictions following covid lockdowns. Application must be made in writing to the owner and provide details of classes not run.

I have read all the above terms and conditions of enrolment, and the information supplied regarding Amy Beccari Dance Studios and hereby agree to all its terms.
